Release Waiver information:

In consideration of the acceptance of this entry, I waive and release any and all claims against Izzie's Gifts of Hope Foundation, any sponsors, cooperating and supporting groups and any individuals supporting this event.

I hold the above individuals and groups harmless for any and all injuries which may result from my participation.  I certify that I am physically able to participate in this event. 

I grant my permission to Izzie's Gifts of Hope Foundation to use my name and/or picture without limitation and obligation of anyone to compensate me further.

SAVE THE DATE

The first fundraiser  for  Izzie's  Gifts  of  Hope Foundation will be held June 5, 2010 at 9:30 am at North Boundary Park in Cranberry Township, PA.  It is called Izzie's Dash for Organ Donation.  It will be a  family  oriented  walk  celebrating  the  "gift of life" while also

raising awareness about the importance of organ donation.
